Local Area Transition Project

Disability_Social_Group

This project was funded by the Department of Human Services (DHS) for an 18 month period to trial a whole of life planning approach for students with a disability. This involved, Project Coordinator, Sarah Every to undertakea whole of life planning for students with a disability as well as connecting the young person with their community, recreational services and social activities with the goal of increasing independence.

The Local Area Transition Project has now come to an end, however we have had some great outcomes for young people including, some gaining work experience, increasing social activities, and some even gaining employment. We have also been able to have some fantastic opportunities for parents and young people, such as setting up social groups (which parents are now running themselves) and running Parents as Career Transition Supports (PACTS) in conjunction with the Cerebral Palsy Support Network.

A number of key findings and recommendations have been developed based on the project evaluation, outcomes documented and feedback on the project.

Recommendations from the project include:

  • An information resource for parents and young people to tap into as needed
  • Partnerships between the school and the community to increase for parents to confidently    explore other options
  • Consistency in the disability sector
  • The need for planning to begin earlier at school
